Purpose: The primary surgical treatment modality for large or complex renal stones is percutaneous nephrolithotomy (PCNL). Cone beam computed tomography (CBCT) allows for intraoperative imaging and can be used to enhance intraoperative assessment of a stone-free status during a PCNL procedure. However, scanning protocols have not yet been optimized for this purpose. The high degree of stone and patient characteristics require a tailored approach. This study aimed to select the most suitable CBCT protocols for imaging stone fragments intraoperatively during PCNL. Methods: A phantom insert with 100 calcifications varying in size and density was placed in an anthropomorphic abdominal phantom. Nine different CBCT protocols were used with varying dose and copper filter settings, and each scan was repeated five times with a small translation between each scan. Detectability of the calcifications was scored by visual assessment and visibility curves were generated for each protocol, depicting the minimum size and density at which calcifications were still detectable. Image noise and contrast-to-noise ratios (CNRs) were calculated for each protocol, as well as estimated effective patient doses per CBCT scan. Results: Calcification detectability and CNRs decreased with a decrease in radiation dose, whereas noise ratios increased. Three suitable scanning protocols were selected, a high-dose or soft-stone CBCT protocol resulting in an effective dose of 10.7 millisievert (mSv) per CBCT scan, a medium-dose CBCT protocol resulting in an effective dose of 4.6 mSv and a low-dose or hard-stone CBCT protocol resulting in an effective dose of 2.2 mSv. Conclusion: Radiation dose for intraoperative CBCT for imaging calcifications can effectively be lowered while maintaining calcification visibility, by implementing low-dose CT protocols with lowered dose settings and the application of a copper filter. The three proposed scanning protocols provide options to select a scan protocol based on stone type and patient characteristics.

Measurement of accurate attenuation of photon flux in tissue is important to obtain reconstructed images using single-photon emission computed tomography (SPECT). Computed tomography (CT) scanner provides attenuation correction data for SPECT as well as anatomic information for diagnostic purposes. Segmentation is a process of dividing an image into regions having similar properties such as gray level, color, texture, brightness, and contrast. Image segmentation is an important tool for evaluation of medical images. X-ray beam used in CT scan is poly-energetic; therefore, we have used a copper filter to remove the low energy X-rays for obtaining correct attenuation factor. Images obtained with and without filters were quantitatively evaluated by segmentation method to avoid human error.

Axial images of AAPM CT phantom were acquired with 3 mm copper filter (low intensity) and without copper filter (high intensity) using low-dose CT (140 kvp and 2.5 mA) of SPECT/CT system (Hawkeye, GE Healthcare). For segmentation Simulated Annealing Based Fuzzy c-means, algorithm is applied. Quantitative measurement of quality is done based on universal image quality index. Further, for the validation of attenuation correction map of filtered CT images, Jaszczak SPECT phantom was filled with 500 MBq of (99m)Tc and SPECT study was acquired. Low dose CT images were acquired for attenuation correction to be used for reconstruction of SPECT images. Another set of CT images were acquired after applying additional 3 mm copper filter. Two sets of axial SPECT images were reconstructed using attenuation map from both the CT images obtained without and with a filter.

When we applied Simulated Annealing Based Fuzzy c-means segmentation on both the CT images, the CT images with filter shows remarkable improvement and all the six section of the spheres in the Jaszczak SPECT phantom were clearly visualized.

This study was undertaken to evaluate the effect of external metal filters on the image quality of computed tomography (CT) and single photon emission computed tomography (SPECT)/CT images. Images of Jaszack phantom filled with water and containing iodine contrast filled syringes were acquired using CT (120 kV, 2.5 mA) component of SPECT/CT system, ensuring fixation of filter on X-ray collimator. Different thickness of filters of Al and Cu (1 mm, 2 mm, 3 mm, and 4 mm) and filter combinations Cu 1 mm, Cu 2 mm, Cu 3 mm each in combination with Al (1 mm, 2 mm, 3 mm, and 4 mm), respectively, were used. All image sets were visually analyzed for streak artifacts and contrast to noise ratio (CNR) was derived. Similar acquisition was done using Philips CT quality control (QC) phantom and CNR were calculated for its lexan, perspex, and teflon inserts. Attenuation corrected SPECT/CT images of Jaszack phantom filled with 444-555 MBq (12-15 mCi) of (99m)Tc were obtained by applying attenuation correction map generated by hardened X-ray beam for different filter combination, on SPECT data. Uniformity, root mean square (rms) and contrast were calculated in all image sets. Less streak artifacts at iodine water interface were observed in images acquired using external filters as compared to those without a filter. CNR for syringes, spheres, and inserts of Philips CT QC phantom was almost similar to Al 2 mm, Al 3 mm, and without the use of filters. CNR decreased with increasing copper thickness and other filter combinations. Uniformity and rms were lower, and value of contrast was higher for SPECT/CT images when CT was acquired with Al 2 mm and 3 mm filter than for images acquired without a filter. The study suggests that for Infinia Hawkeye 4, SPECT/CT system, Al 2 mm, and 3 mm are the optimum filters for improving image quality of SPECT/CT images of Jaszack or Philips CT QC phantom keeping other parameters of CT constant.

Cone beam computed tomography (CBCT) has often been used to determine the quality of craniofacial bone structures through the determination of mineral density, which is based on gray scales of the images obtained. However, there is no consensus regarding the accuracy of the determination of the gray scales in these exams. This study aims to provide a literature review concerning the reliability of CBCT to determine bone mineral density. The gray values obtained with CBCT show a linear relationship with the attenuation coefficients of the materials, Hounsfield Units values obtained with medical computed tomography, and density values from dual energy X-ray absorciometry. However, errors are expected when CBCT images are used to define the quality of the scanned structures because these images show inconsistencies and arbitrariness in the gray values, particularly when related to abrupt change in the density of the object, X-ray beam hardening effect, scattered radiation, projection data discontinuity-related effect, differences between CBCT devices, changes in the volume of the field of view (FOV), and changes in the relationships of size and position between the FOV and the object evaluated. A few methods of mathematical correction of the gray scales in CBCT have been proposed; however, they do not generate consistent values that are independent of the devices and their configurations or of the scanned objects. Thus, CBCT should not be considered the examination of choice for the determination of bone and soft tissue mineral density at the current stage, particularly when values obtained are to be compared to predetermined standard values. Comparisons between symmetrically positioned structures inside the FOV and in relation to the exomass of the object, as it occurs with the right and left sides of the skull, seem to be viable because the effects on the gray scale in the regions of interest are the same.

In SPECT, the sinogram contains scatter and lack of attenuated counts that degrade the reconstructed image quality and quantity. Many techniques for attenuation and scatter correction have been proposed. An acceptable method of correction is to incorporate effects into an iterative statistical reconstruction. Here, we propose new Maximum Likelihood Expectation Maximization (MLEM) formula to correct scattering and attenuating photons during reconstruction.

In this work, scatters are estimated through Klein-Nishina formula in all iterations and CT images are used for accurate attenuation correction. Reconstructed images resulted from different MLEM reconstruction formula have been compared considering profile agreement, contrast, mean square error, signal-to-noise ratio, contrast-to-noise ratio and computational time.

The proposed formula has a good profile agreement, increased contrast, signal-to-noise (SNR) & contrast-to-noise ratio (CNR), computational time and decreased mean square error (MSE) compared with uncorrected images and/or images from conventional formula.

In conclusion, by applying the proposed formula we were able to correct attenuation and scatter via MLEM and improve the image quality, which is a necessary step for both qualitative and quantitative SPECT images.
