EZH2在肝脏疾病中的研究进展

Zeste基因增强子同源物2(enhancer of zeste homolog 2, EZH2)在一些恶性肿瘤组织中呈现过度表达, 与肿瘤的生长、发展、预后有着密切的关系. 最新研究表明, EZH2在肝脏疾病领域除了与肝癌的发生、进展相关, 在免疫方面也发挥着不可忽视的作用, 其参与许多肝脏疾病的发生、发展, 与慢性乙型肝炎的病毒复制、非酒精性脂肪性肝病的炎性反应、慢性肝炎肝纤维化的进展均有着密切的关系, 可能成为今后肝病研究领域的重要方向. 本文就最近十年来对EZH2在肝脏疾病中的研究进展及其在治疗中的潜在应用价值进行综述.

Overexpression of enhancer of zeste homolog 2 (EZH2) protein has been found in several malignant tumor tissues and is closely related to the degree of tumor differentiation, clinical stage, tumor size, and prognosis. The latest research shows that overexpression of EZH2 is related not only with immunoregulation but also with the replication of hepatitis B virus, the occurrence of non-alcoholic fatty liver disease, and the progress of liver fibrosis, which may become an important subject in the field of liver disease research. This paper reviews the role of EZH2 in some liver diseases and its potential application in treatment of these diseases in recent ten years.

0 引言

Zeste基因增强子同源物2(enhancer of zeste homolog 2, EZH2)是果蝇zeste基因增强子的人类同源物, 属于PcG(polycomb group, PcG)基因家族[1], 是构成多梳抑制复合体2(polycomb repressive complex 2, PRC2)的三个亚单位之一[2]. EZH2基因共有四个高度保守区域, 位于N端的Ⅰ区、Ⅱ区和富含半胱氨酸区及位于C端的SET区域. EZH2的组蛋白甲基转移酶(histone methyltransferases, HMT)主要是通过SET结构域对核小体组蛋白H3的第9位和第27位赖氨酸残基进行甲基化而发挥作用, 因此, SET结构域的完整性对EZH2基因发挥转录抑制功能具有极其重要的作用. 当EZH2基因缺失SET结构域后, 则不能产生抑制表型, 甚至可能会导致基因去抑制化[3].

EZH2可以调控HMT、组蛋白去乙酰化酶(histone deacetylase, HDAC), 导致肿瘤细胞基因表达异常[4]. EZH2与胚胎外胚层发育蛋白(embryonic ectoderm development, EED)形成复合物, 可以直接与HDAC作用, 调节部分沉默的组蛋白成分, 最终导致肿瘤的发生[5]. 在很多恶性肿瘤组织中, EZH2均呈现过度表达, 因此其与恶性肿瘤的形成、侵袭、转移和预后有密切的关系. 在促进细胞增殖方面, Visser等[6]最早在淋巴细胞瘤增殖细胞中发现EZH2过度表达, 而在静止的细胞中表达受到抑制, 甚至不表达, 因此提出EZH2参与了细胞的生长和增殖. 通过转染siRNA技术抑制EZH2的表达, 或使用RNAi技术敲除EZH2基因, 可以导致细胞周期停滞于G1, G2和G2/M期[7], 进一步证实了EZH2在细胞增殖中的作用. 最新研究还表明, EZH2表达下调能显著促进机体Th1和Th2细胞的免疫应答及炎症反应[8], 也可加剧NAFLD患者机体内脂肪堆积和炎症反应[9], 在α干扰素(interferon- alpha, IFN-α)介导的HBV cccDNA转录抑制过程中也发挥重要的作用[10]. 另外, EZH2可以激活Wnt/β-catenin信号通路, 通过抑制Dickkopf-1(DKK1)促进肝星状细胞的活化和增殖, 导致肝纤维化的进展[11]. 本文就EZH2在肝病免疫及各类肝病发生、发展过程中的作用做一综述.

1 EZH2与肝脏免疫

EZH2是特异性催化H3K27me3的组蛋白甲基转移酶[12], 对维持细胞记忆和转录起着非常关键的作用[13]. 研究表明, 在CD4+ CD8+双阳性胸腺细胞中, EZH2的缺失对外周淋巴组织成熟T细胞的分化和效应功能没有影响[14]. 在幼稚T细胞中EZH2呈低水平表达, 当受到异体抗原刺激后, CD4+和CD8+细胞中的EZH2迅速上调, 呈高表达状态[15] .

有研究表明, EZH2 通过TBX21和GATA3等控制Th1和Th2细胞的分化[8,16]. EZH2与IL4启动子区结合[17], 通过对粒细胞因子(如STAT6和GATA3)转录的抑制对Th1和Th2分化起到调控作用. IFN-γ表达依赖于转录因子, 当Th2细胞缺失STAT6后, IFN-γ的表达较野生型Th2细胞明显增高[18]. 在Th17, Th9和iTreg细胞的诱导下, EZH2缺失的Th2细胞IFN-γ的表达较野生型Th2细胞明显增高[8]. 当GATA3异位表达时, 在Th2细胞中可以发现有更多的EZH2结合于IFNG启动子区域[18]. 新近研究表明, EZH2与IL17的表达呈正相关[19]. 目前EZH2的缺失是否与T细胞的分化异常相关尚无定论, EZH2在效应T细胞不同亚群中的作用也值得进一步探究.

动物实验证明, EZH2的缺失对CD4+T细胞IL-4的表达以及Th2相关基因(如Stat6、Gata3、IL-4、IL-5、IL-13)的表达没有显著影响, 但会导致Th1相关基因(Stat4, TBX21和Ifng)的表达明显下降[20]. 而Th1细胞主要与慢性感染相关, 并且有研究表明, Stat4、TBX21也与自身免疫性肝炎发病相关[21,22]. 在针对慢性丙型肝炎患者的研究中, Stat4, TBX21和Ifng与丙型肝炎病毒的清除与免疫耐受均相关[23,24]. 因此, EZH2的缺失导致的免疫紊乱可能与自身免疫性肝病、慢性病毒性肝炎、肝衰竭发病相关, 其中的机制、作用有待进一步研究.

2 EZH2与肝癌

肝癌是我国常见的消化系统肿瘤, 每年约有30万例新发病例, 其病死率排名第三. 肝癌发病隐匿、进展迅速, 且易发生转移, 寻找与肿瘤发生、发展相关的肿瘤标志物是提高肝癌治疗效果的关键.

EZH2可以激活干细胞特异基因, 并调控抑制p16、p27、miRNA和BRCA1等抑癌基因, 在多种恶性肿瘤组织中过度表达, 如淋巴瘤、大肠癌、肝癌、前列腺癌、乳腺癌、膀胱癌等, 与恶性肿瘤的形成、侵袭、转移和预后有密切的关系[25]. 动物实验发现, 在小鼠模型中, 通过抑制 EZH2的活性能抑制甚至完全阻断肿瘤的生长[26]. 对抑癌基因p53的研究表明, 激活的p53可以抑制EZH2基因的表达, 使细胞周期停滞在G/M期[27]. 用转染小干扰RNA(siRNA)或小发夹RNA(shRNA)同样可以抑制EZH2的表达, 使细胞周期在G1、G2和G2/M停滞[28]. Yang等[29]发现, 长链基因间非蛋白质编码RNA在肝癌细胞5′呈高表达, EZH2与其共同介导, 在p16、p27等基因的启动子区域催化组蛋白过度甲基化, 导致p16、p27等表达水平下降, 解除G0/G1期阻滞, 促进了肝癌细胞的增殖. 在肝癌细胞株的研究中, 发现EZH2可以通过表观调控抑制miR-22, 从而下调了galectin-9的表达, 促进了肿瘤的生长和转移[30].

表观遗传的改变是肿瘤的重要标志之一, 异常的组蛋白修饰可导致抑癌基因的沉默. Cai等[31]发现, 三甲基化H3K27在肝癌组织中呈高表达, 表达越高的肝癌患者预后越差. EZH2通过与DNA甲基转移酶(DNA methyltransferase 1, DNMT1)的协作, 使靶基因CpG岛区DNA甲基化, 从而导致靶基因的永久沉默, 促进肿瘤细胞的增殖、侵袭甚至凋亡. 在一些恶性肿瘤的研究中, 发现EZH2可通过抑制靶基因Ecadherin的表达, 调控肿瘤转移能力[32]; EZH2可通过抑制促凋亡基因BIK和NOXA的表达来减弱乳腺癌细胞的凋亡反应, 从而促进化疗耐药[33]; EZH2还使IGTA2表达沉默, 从而促进结直肠癌的上皮-间质转换[34]. 因此, 如何利用对EZH2的调控, 开发治疗肝癌的新方法值得进一步深入研究.

3 EZH2与慢性乙型肝炎

乙型肝炎病毒(hepatitis B virus, HBV) 共价闭合环状DNA(covalently closed circular DNA, cccDNA)是乙肝病毒复制、转录的原始模板. 目前慢性乙型肝炎患者通过使用干扰素或者核苷(酸)类似物抗病毒药物治疗, HBV-DNA得到持续的抑制, 甚至出现了HBeAg血清学转换, 但是由于肝细胞核内HBV cccDNA难以被清除, 所以一旦停止使用抗病毒药物, 病毒复发大多会再次发生. 因此如何清除HBV cccDNA, 达到慢性乙型肝炎的临床治愈是近年来研究的热点.

IFN-α主要通过免疫调节进行抗病毒治疗, 同时, 也有一定的直接抗病毒活性. 有研究表明, IFN-α可以影响HBV cccDNA微染色体的表观遗传. Belloni等[10]用IFN-α处理HBV/HepG2细胞48 h后, 发现cccDNA结合组蛋白H4发生去乙酰化, PRC2家族的EZH2、HDAC1、hSirt1, 和转录因子YY1的表达明显上调, 抑制了cccDNA的转录. 进一步的研究表明, 在IFN-α治疗停止后48 h, EZH2的表达保持不变. 因此, 前基因组RNA(Pregenome RNA, pgRNA)转录和细胞质HBV核心颗粒的产生仍持续受到抑制. 研究提示了EZH2在IFN-α介导的HBV cccDNA转录抑制过程中起到了重要的作用, 可能有助于在分子机制上进一步探究新的抗病毒治疗靶点.

4 EZH2与NAFLD

NAFLD是最常见的慢性肝病之一. 近年来NAFLD的发病率急剧上升, 尤其在儿童中的患病率也呈逐年上升趋势, 越来越受到人们的关注[35,36]. NAFLD有着特定的遗传背景(即特定的基因多态性存在)和环境因素(如高热量饮食习惯、久坐不动的生活方式)[36,37], 除了上述这些致病因素, 表观遗传机制也可能是导致NAFLD发生、发展的相关因素[38,39]. 有研究表明, 过多的营养摄入可能导致表观遗传一些特定的基因发生改变, 从而使个体变得更容易患与饮食相关的疾病, 如代谢综合征和NAFLD[40,41]. 但是除了研究最广泛的miRNAs, 其他表观遗传机制在NAFLD发病机制中的作用仍知之甚少[38,39,42]. DNA甲基化的调节已被证明是编码参与人体胰岛素抵抗和炎症的蛋白质相关基因[43]. 此外, 小鼠实验表明, 脂肪肝的发展与表观遗传基因的突变相关[43]. 最近, Pirola等[44]研究发现, 肝线粒体DNA的胞嘧啶甲基化与NAFLD的组织学损伤严重程度有关.

Vella等[9]研究表明, EZH2基因或功能性突变以及基因多态性相关的下调可能在人类NAFLD的发生、发展过程中起到重要作用. 在NAFLD大鼠的肝脏和游离脂肪酸处理HepG2细胞株中, EZH2蛋白表达与活性均下降, 且与以下因素成负相关: (1)脂质堆积程度; (2)炎性标志物包括TNF-α、TGF-β的表达; (3)miR-200b和miR-155的表达. 此外, 在上述实验中使用EZH2抑制剂3-Deazaneplanocin A(DZNep)后可显著降低EZH2的表达及活性, 同时脂质聚集、炎症分子和微小RNA的表达均明显增加. 这些研究证明, EZH2的下调加剧NAFLD机体内脂肪堆积和炎症反应. DZNep可以通过直接上调EZH2控制表达的蛋白或miRNA, 或作用于其他PRC2核心蛋白, 如EED和SUZ12, 影响肝脏的脂质沉积和炎症变性[45]. 由于NAFLD的疾病谱比较宽泛, 包括单纯性脂肪肝、脂肪性肝炎、脂肪性肝纤维化、肝硬化和肝癌, 因此, 关于EZH2在NAFLD中的作用, 今后还需进行更深入的研究.

5 EZH2与肝纤维化

肝纤维化是世界范围内困扰学术界的医学难题之一, 最终导致终末期肝病, 其发病率高, 死亡率高. 肝星状细胞在肝纤维化的发生、发展过程中起到关键性作用. 正常情况下, 肝星状细胞(hepatic stellate cells, HSCs)处于静止状态. 当肝脏受到炎症或机械刺激等损伤时, 肝星状细胞被激活, 其表型由静止型转变为激活型, 高度增殖, 并转化为肌成纤维细胞样细胞(myofibroblasts, MFB). 当HSCs转变成MFB时, 细胞增殖加速, 细胞外基质(extracellularmatrix, ECM)的合成显著增多, 导致了肝纤维化的不断进展. 因此, 阻断HSCs活化、增殖是控制肝纤维化的重要治疗策略[46,47]. 然而, 控制HSCs增殖和活化的机制仍不清楚[48,49]. 研究证明, Wnt/β-catenin信号通路在细胞增殖中起着重要作用, 是肝纤维化的发生并持续进展的因素之一[50]. Wnt/β-catenin信号通路能被转化生长因子-β1(transforming growth factor-β1, TGF-β1)诱导的HSCs激活[51,52]. 敲除β-catenin后可以抑制细胞增殖和活化的HSCs中α-平滑肌肌动蛋白(a-smooth muscle actin, α-SMA)的表达[53]. 此外, Dickkopf家族(Dickkopf family, DKK)的过度表达, 尤其是Wnt/β-catenin信号拮抗剂DKK-1, 能够抑制原代培养HSCs的生长[54].

Yang等[11]在四氯化碳诱导的小鼠肝脏纤维化模型实验中发现, 模型小鼠的肝脏内β-catenin表达增加. 通过siRNA敲除DKK1可增加β-catenin的表达和促进HSCs的活化. 进一步研究证实, DKK1是HSCs分化过程中的转录抑制因子. 以往的研究表明, 在肺癌研究过程中发现EZH2可以通过H3K27me3甲基化抑制DKK1转录. Yang等[11]在四氯化碳诱导的小鼠肝纤维化模型中, 证实模型鼠肝脏内HSCs及TGF-β1诱导的HSCs中EZH2表达水平均增高. 通过DZNep抑制EZH2以后, 细胞活力下降并阻止了HSCs活化. siRNA敲除EZH2基因后也可以阻止HSCs活化. 在敲除EZH2基因的HSCs中也得到了相似的结果. 同样, 敲除EZH2基因可以抑制TGF-β1诱导的HSCs中β-catenin的表达, 使HSCs活化. 这个实验首次证明了EZH2可以通过抑制DKK1激活Wnt/β-catenin信号通路, 是体外和体内HSCs活化的一个关键调节因素. 因此, EZH2是否可以作为肝纤维化治疗方面的靶点值得进一步研究.

6 结论

总之, EZH2作为表观遗传学研究中一个重要的组蛋白甲基转移酶, 在免疫方面发挥着不可忽视的作用, 参与许多肝脏疾病的发生发展, 其作用日益得到人们的重视. 目前, 体外实验发现EZH2可能与IFN-α抑制HBV cccDNA复制有着密切的关系, 动物实验发现EZH2与NAFLD、肝纤维化发病机制也有一定的关联性, 对其作用机制有了一定的认识. 但许多方面的具体机制还不十分明确, 是否能成为肝病治疗的新靶点有待进一步深入研究. 同时, 在自身免疫性肝病及其他原因导致的慢性肝病、甚至肝衰竭方面的作用也有进一步的研究价值. 随着对EZH2基因在肝病方面的进一步探索, 相信研究成果有可能为今后肝病的防治带来新的方法.
