肠易激综合征(irritable bowel syndrome, IBS)是最常见的一种功能性疾病, 病因尚不清楚, 先前的研究表明其发病机制与下列因素有关: 胃肠动力学异常, 内脏感觉异常, 精神因素, 神经递质和肠道菌群失衡, 肠道感染以及食物不耐受等因素. 而近来遗传因素和IBS发病关系的研究也进一步深入, 本文综述遗传和相关基因多态性与IBS发病之间的联系.

Irritable bowel syndrome is a common functional disorder, whose etiology remains unknown. Previous researches indicated that its pathogenesis might be associated such factors as gastrointestinal motor abnormality, altered visceral sensation, psychological factors, neurotransmitter and imbalance of gastrointestinal flora, intestinal infection and food intolerance, etc. Besides, recent research has touched on relationship between genetics and IBS. This paper reviewed relationship between the inheritance, gene-linked polymorphism and irritable bowel syndrome pathogenesis.

0 引言

肠易激综合征(irritable bowel syndrome, IBS)是一种以腹痛或腹部不适伴排便习惯改变为特征的功能性肠病. 既往分别在胃肠动力[1], 内脏敏感性[2], 精神心理因素[3-4], 肠道菌群失调[5-8], 肠道感染[9]和食物不耐受等相关因素方面进行了较多的研究. 因其病因及发病机制不明确, 使其治疗方面也存在一定的难度. 遗传学及相关基因与IBS病因及发病机制的关系也逐渐被关注与研究.

1 遗传学和发病机制

Morris-Yates et al研究显示[12], 对686例双胞胎进行问卷调查, 其中33例(4.8%)有一个或者多个胃肠道症状, 功能性肠病的一致性在单卵双胞胎是33%, 而在双卵双胞胎是12%(P<0.05), 此研究说明功能性肠病的发病可能受遗传控制. Lecy et al[13]对10, 699例进行了问卷调查, 其中6060个双胞胎, 同时对其父母是否也患有IBS进行了调查, 问卷结果是单卵双胞胎有17%的一致性, 双卵双胞胎8%的一致性, 这一结果进一步支持了遗传因素在IBS发病中起到重要作用. 因此, 近来对遗传因素与IBS发病关系进行了大量的研究.

动力、分泌及内脏感觉改变是肠易激综合征的特征, 在消化道黏膜层肠嗜铬细胞和感觉神经之间的5羟色胺(serotonin, 5-HT)信号通路调节胃肠道功能, 发现5-HT信号通路的改变有可能参与IBS患者肠道功能及感觉改变的发病机制[14-21]. IBS的5-HT信号通路改变的要素包括: 肠嗜铬细胞数量、色氨酸羟化酶信号水平、血清5-HT水平和5-HT再摄取转运体的表达等. IBS 患者中先天的和环境因素均可导致5-HT再摄取转运体水平的降低, 5-HT选择性再摄取转运体基因启动子区多态性导致了患者的遗传易感性, 而炎症介质可诱发5-HT再摄取转运体水平的下调[22]. 当然, 仍需进一步的研究以获取更全面的了解5-HT信号通路的改变, 他们的诱因与影响以及其中何种变化有病理生理学的意义.

2 5-HT转运体多态性
2.1 与IBS的关系

国内外部分研究表明5-HT遗传因素可能与IBS的发病相关[23-28], 人类5-HT转运体基因存在多个多态性位点. 国外研究显示启动子区的5-HT转运体基因相关多态区(5-HT gene-linked polymorphic region, 5-HTTLPR)和内含子二区的可变数串联重复序列(variable number tandem repeats, VNTRs)多态性与IBS相关. Saito et al[29]一项遗传药理学的研究比较IBS与正常对照者5-HTTLPR和GNbeta3-C825T等位基因多态性的基因频率区别, 所有调查对象均完成调查问卷和抽血, PCR方法检测DNA分型. 结果显示混合型IBS中5-HT转运体基因的相关多态区中S等位基因出现频率高于正常对照组, 从而认为5-HTTLPR与混合型IBS发病有关, 而与腹泻型IBS和便秘型IBS发病无明显关联. GNbeta3-C825T的基因多态性与所有类型IBS患者无明显联系. Wang et al[30]也研究了5-HT转运体多态性与肠易激综合症的关系, 他们使用PCR方法测定81例IBS患者与48例正常对照者5-HT转运体基因内含子二区VNTRs和5-HTTLPR的区别, 结果发现相对于正常对照者, IBS患者的VNTRs区有较高频率的STin2.12/10基因型和较低频率的STin2.12/12基因型, 而这个区的基因多态性在便秘型IBS, 腹泻型IBS和混合型IBS之间无明显差异. 5-HTTLPR等位基因L/L基因型在便秘型IBS患者中出现频率高于腹泻型和混合型IBS患者, 同时L/S基因型出现频率降低, 另有便秘型IBS中VNTRs中STin2.12/12基因型和5-HTTLPR中L/L基因型关联出现的频率是高于混合型IBS和正常对照者的. Yeo et al[31]以194例北美白种女性IBS患者和448例白种女性健康对照为对象, 以9种5-HT转运体多态性为基础, 对所有受试者DNA进行PCR分析, 也同样发现5-HT转运体是女性腹泻型IBS的一种潜在的疾病侯选基因. Li et al[32]探讨5-HT转运体与IBS发病的关系中, 用PCR方法检测使用罗马Ⅱ标准诊断的87例IBS患者和96例正常对照者的5-HT转运体基因多态性, 结果也同样显示5-HTTLPR等位基因L/L基因型出现频率在便秘型IBS患者显著高于对照组. 谭慧珍 et al[33]和谢军 et al[34]的研究也同样显示. 5-HT转运体多态性与IBS患者抑郁和焦虑症状的关系亦有报道. Jarrett et al[35]研究了两种精神症状与两种5-HT转运体多态区: 分别是位于启动子区5-HTTLPR和内含子二区可变数串联重复序列(STin2 VNTR), 对21例男性患者和117例女性患者, 均行精神健康史和现在的心理不良应激评估, 并取血做基因分型. 结果发现存在纯合的短等位基因5-HTTLPR或携带有STin2 VNTR等位基因的患者较其他更易有抑郁, 焦虑病史和一般的心理不良应激, 这个结果支持了IBS的生物-心理-医学模式, 也支持了5-HT转运体基因型可以改变抑郁发作的风险.

2.2 调节5-HT转运体多态性的相关细胞因子

有多项研究显示肠道黏膜5-HT转运体在结肠炎动物模型、炎症性肠病和IBS患者肠道黏膜中表达是降低的, 而5-HT转运体的功能和表达的改变可能是上述疾病中肠道功能紊乱(动力改变、黏膜分泌以及感觉异常)的发病基础. 为了能找出引起5-HT转运体表达下调的调节因子, Foley et al[36]选用了来源于有活性的人淋巴细胞作为条件培养基, 培养结肠上皮细胞(Caco2, 这种培养基促成了5-HT再摄取), 培养的细胞分别被IFN-γ(500 mg/L), TNF-α(50 mg/L), IL-12(50 mg/L)处理, 经过3 d的培养, 结果发现IFN-γ和TNF-α导致了5-HT运载体功能的降低, 这两种细胞因子也使得5-HT运载体mRNA和蛋白水平降低.

3 GNbeta3-C825T基因多态性

GNbeta3通过配体激活作用影响G蛋白翻译, 已有报道说明功能性消化不良与GNbeta3基因的纯合基因型C825T多态现象有关[37], 而与下消化道功能紊乱的关系还不明确. Andresen et al[38]研究比较GNbeta3基因型的发病在功能性胃肠病患者与正常对照者之间的不同, 并检测GNbeta3-C825T在IBS, 上腹痛综合征和IBS合并上腹痛综合征, 以及上消化道症状评分高低患者之间遗传性的不同, 结果发现GNbeta3-C825T基因多态现象与下消化道功能紊乱疾病之间并无关系. 这一点在Saito et al[29]的研究中也同样显示. 另有Cao et al[39]研究提示GNbeta3-C825T多态性与抑郁障碍存在明显的相关性, 认为GNbeta3-C825T等位基因增加了抑郁障碍患病风险.

4 TNF-α和IL-10基因多态性

炎症可能在部分IBS患者发病机制中起到一定作用, 持续的炎症反应, 起因于细胞因子调节炎症反应的不平衡, 而细胞因子受基因控制已经确立, 那么IBS患者中是否存在特有的遗传素质所致的抗炎细胞因子产生的改变, 很多学者对此进行了研究. Frieling et al[40]的研究显示炎症可致IL-10启动子区基因多态性的变化. Gonsalkorale et al[41]分别从230例IBS患者和正常对照者的外周血细胞因子中提取DNA, 通过对IL-10的-1082区的检测分析, 结果发现IBS患者有明显的高表达IL-10水平基因型的减少. 这一结果提示了至少部分IBS患者存在抗炎因子IL-10的量较正常对照低的遗传素质. 另有van der Veek et al[42]研究了TNF-α和IL-10基因多态性和IBS发病的关系, 具体针对IBS患者TNF-α和IL-10的基因启动子区单核甘酸多态性, 通过分别从IBS患者和正常对照的外周白细胞中提取DNA, 分析单个核甘酸TNF-α的-308和-1082的单核甘酸多态区和IL-10的-819单核甘酸多态区的基因型和等位基因频率, 结果发现纯合子高表达TNF-α比较少见, 杂合子TNF-α基因型(G/A)IBS患者显著高于正常对照者, 更多的患者相对于对照组表现为A等位基因的高阳性率, 且同样的倾向出现在腹泻型IBS与其余两种亚型相对比的情况下. IL-10基因型在患者和正常对照组单个核甘酸多态区同样分布, IBS患者中拥有高表达TNF-α和低表达IL-10基因型发生率较对照组高, 在腹泻型IBS较便秘型和混合型IBS患者高. Wang et al[43]也研究了IL-10基因启动子区域的-1082, -819, -592位置的三倍体等位基因与腹泻型IBS的关系, 他们利用放大受阻突变体系PCR方法扩增出IL-10基因启动子区域的-1082, -819, -592, 比较其腹泻型IBS患者与正常对照者的区别, 发现腹泻型IBS患者IL-10基因启动子-819区有较高频率的T/T基因型以及-592区A/A基因型出现频率增加, 均较健康对照者有显著性差异. 其结果进一步支持了遗传决定免疫反应在IBS发病中存在作用的新设想, 更多的研究须解决哪些确定的细胞因子基因多态性与IBS各种亚型的关系.

5 á2-肾上腺素能受体与IBS

胃肠道受外周去甲肾上腺素能神经支配来调节各种消化功能[44]: 包括黏膜分泌, 肠道蠕动和消化道感觉, 并通过á2-肾上腺素能受体活动, 此类受体几乎参与了所有肠道神经递质接头前调节, 他们也同样在突触后间隙起到作用. 临床研究显示á2-肾上腺素能受体在结肠蠕动幅度和感觉方面均有作用, 且用á2-肾上腺素能受体激动剂可以改善肠道功能诱导IBS患者症状缓解. 而遗传研究表明á2-肾上腺素能受体基因多态性在功能性下消化道疾病中与便秘及躯体症状有着明显联系. 有证据显示交感肾上腺素能功能障碍与IBS的发病也有一定关系[45]临床术后肠梗阻模型表明在肠肌丛单核细胞中á2-肾上腺素能受体强烈表达, 该受体通过阻碍肠道神经传递, 促进该区黏膜炎症反应, 从而致术后消化道功能异常. 因此认为á2-肾上腺素能受体在肠道炎症反应过程中是上调的, á2-肾上腺素能受体调节机制所致的动力改变在炎症和非炎症区都是存在的, 此项研究为今后在针对á2-肾上腺素能受体靶向药物治疗IBS提供了可能性的前提. 人类的á2-肾上腺素能受体分为α2A, α2B和α2C[46-47]. Kim et al[48]研究结果显示两种不同的α2A和α2C肾上腺素受体与便秘型IBS及高的躯体症状评分患者有关.

6 结论

IBS与遗传学存在着密切的关系, 其中5-HT转运体基因多态性在IBS发病中起着很重要的作用, IBS患者中5-HT转运体STin2.12/10基因型的出现可能与IBS发病有关, 而其基因联合多态区等位基因L/L基因型和其与STin2.12/10基因型关联出现增加了便秘型IBS的发病率, 而L/S基因型的出现增加了腹泻型-IBS的发病率[49]. 5-HT转运体基因多态性也与IBS患者抑郁焦虑症状的发生也存在密切关系. 虽然在一项5-HT转运体蛋白活动中基因编码的功能性多态现象与IBS关系的Meta分析中, Van Kerkhoven et al[50]通过描述5-HT产生长链和短链等位基因多态现象的频率, 评价了5-HT转运体蛋白活动中基因编码的功能性多态现象与IBS的关系, 8个被选的研究描述了总共1034例IBS患者及1377例正常对照者短链等位基因的发生与IBS发病风险增加并无联系. Lee et al[51]的研究也同样认为总体来说两者之间并无明显联系. 尽管如此, 大量的实验还是为5-HT转运体基因多态性与IBS的发病关系提供了肯定的依据, 仍需进一步的研究来证实其中具体的联系以及机制.
