终末期肝病的发病率很高, 在亚洲尤其突出, 肝移植是其治疗的有效手段. 如何诱导肝移植免疫耐受是免疫学工作者及临床医师遇到的免疫学理论和临床现实问题. 细胞凋亡是近10年来研究的热点问题. 本文主要介绍细胞凋亡的概念, 形态学的改变, 检测方法, 肝移植免疫耐受的特殊性, 细胞凋亡与肝移植免疫耐受、排斥反应的关系等研究现状. 强调了T细胞凋亡诱导肝移植免疫耐受.

0 引言

细胞凋亡是细胞和组织的生理学基础, 是细胞在生理状态下特征性死亡形式, 在移植免疫中发挥极其重要的作用. 细胞凋亡与肝移植排斥及免疫耐受之间关系密切, 深入研究细胞凋亡与肝移植免疫的作用机制, 将对解决肝移植的免疫理论问题, 提高肝移植长期存活率具有重大的理论和实际意义.

1 细胞凋亡

细胞凋亡(apoptosis), 是由多基因控制的细胞自主性死亡方式. 1972年英国的Kerr首先提出凋亡的概念[1]. 细胞凋亡与调控机体发育, 维护内环境稳定、肿瘤、病毒感染、免疫密切相关. 有文献称细胞凋亡又叫程序化细胞死亡(programmed cell death, PCD). 实际上二者并不完全相同, 前者主要是形态学概念, 而后者强调的是功能性概念.

1.1 细胞凋亡形态学改变

细胞凋亡的形态变化与坏死截然不同, 主要表现在: (1)细胞核内染色质浓缩形成不均一点状结构; (2)细胞膜微绒毛及细胞间连接复合体消失; (3) 内质网、高尔基体及核膜膨大形成泡状结构与细胞膜融合; (4)可见典型的凋亡小体: 他由细胞膜包被结构尚完整的细胞器(线粒体等)及核碎片形成, 并很快被邻近细胞吞噬[2]. 以上变化可通过染色, 在光镜、荧光显微镜及电镜下观察到, 其中电镜是定性检测细胞凋亡的可靠手段之一. 但形态学观察方法均很难准确鉴定和量化凋亡的发生情况.

1.2 生物化学特征

(1)凋亡的细胞内Ca2+浓度增高. (2)Ca2+、Mg2+依赖性内源性核酸内切酶活化引起的DNA降解, 这是细胞凋亡的显著特征, 是确证细胞凋亡的重要依据.

1.3 检测方法

目前细胞凋亡的常用检测方法有: (1)形态学方法如光镜、电镜; (2)电泳法. (3)免疫学方法. (4)分子生物学方法如: 原位缺口翻译法(ISNT)、脱氧核糖核苷酸末端转移酶介导的缺口末端标记技术(TUNEL)、流式细胞术(FCM)等. 以上方法均为细胞凋亡的研究提供了良好工具[3].

1.4 细胞凋亡的基因调控

随着现代免疫学及分子生物学技术的不断发展, 细胞凋亡的基因调控研究逐渐深入. 近期研究表明: 许多信号途径参与细胞凋亡的调节, 其中死亡受体途径中的Fas/FasL信号途径占据重要地位. Fas属于肿瘤坏死因子受体(TNFR)和神经生长因子受体(NGFR)超家族, 按人白细胞分化抗原分类又称CD95, 与APO-1抗原为同种蛋白质. Fas抗原的天然配体FasL属于TNF家族成员[4]. Itoh et al[5]研究证实: Fas是由325个氨基酸(aa)组成的Mr 45 000的I型膜蛋白, 其膜内区有一段aa序列与TNFR膜内区高度同源, 具有传导"死亡信号"的作用, 称为死亡结构域. FasL是Mr 40 000的II型膜蛋白, 其胞外区约有150 aa与TNF家族成员同源. Fas蛋白表达于心脏、肝脏、肺脏、睾丸等多种组织中; 另外, 激活的成熟淋巴细胞以及感染的淋巴细胞Fas表达增高, 人体部分肿瘤细胞也有Fas表达[6]. 最初认为FasL仅表达在活化的T细胞上, 且Th1克隆的表达高于Th2克隆. FasL及其可溶形式sFasL均可与Fas阳性靶细胞结合, 通过一系列级联反应, 最终导致Fas阳性靶细胞凋亡. sFas与FasL虽然有很高的亲和力, 可同膜型Fas竞争结合FasL, 但不引起凋亡, 起到间接抑制凋亡的作用; sFas和sFasL的作用也可能是体内细胞凋亡的一种调节方式[7].

除Fas家族外, 细胞凋亡还受Bcl-2家族、c-myc家族、p53、白介素1β转换酶(ICE)等多种凋亡相关基因的调控. 在Bcl-2家族中, Bcl-2及Bcl-xl抑制细胞凋亡, 而Bax和Bad可促进细胞凋亡, 并发现Fas系统诱导细胞凋亡与Bcl-2的低表达相偶联; c-myc癌基因的表达既能促进细胞的增生, 也能诱导细胞凋亡; p53是一种抑癌基因, 可通过抑制Bcl-2转录促进Bax转录发挥促细胞凋亡作用; ICE可激活白介素1β导致细胞凋亡, 其作用可受Bcl-2基因的控制. 另外, 干扰素(IFN)、白介素(IL)、肿瘤坏死因子(TNF)等细胞因子也参与细胞凋亡的调控[8].

2 肝移植免疫耐受

肝脏是移植免疫中的特殊器官, 具有"肝免疫耐受性", 肝移植排斥反应的发生率及程度较其他器官移植低[9]. 在部分大鼠[10]和猪[11]及许多小鼠 = 的肝移植模型中, 在不用任何免疫抑制剂的情况下也能够成功诱导供者特异性耐受. 在狗、狒狒和人的肝移植中排斥反应较易控制. 肝移植还可以用来保护其他器官移植, 如肾移植、心脏移植, 显著延长移植物的存活时间[13]. 但肝脏与角膜和睾丸的免疫赦免性不同, 肝移植排斥反应较常见. 肝仅为"免疫特惠"器官. 肝移植的免疫耐受具有其独特性, 为外周的分离耐受[14]. 肝移植免疫耐受的形成可能与肝脏的组织解剖学特点[15]、可溶性MHCⅠ类抗原[16-17]及细胞因子[18-19]的作用、嵌合状态的形成[20-21]、肝脏未成熟树突状细胞与枯否氏细胞的作用[22-23]、调节性T细胞的作用[24-25]及细胞凋亡等相关.

3 调亡细胞与肝脏免疫耐受
3.1 免疫耐受中存在细胞凋亡

1996年Griffith et al[26] 研究向眼前房注射抗原后细胞死亡与免疫耐受诱导的关系, 指出在赦免器官的免疫耐受中CD95介导了淋巴细胞的凋亡; 免疫赦免不是一个被动过程, 而是导致细胞死亡和免疫耐受的主动过程. 不仅在免疫耐受中细胞凋亡存在, 而且, Qian et al[27]发现用MHC不相配小鼠行肝移植, 受鼠自然的接受移植肝, 注射IL-2诱导排斥反应后, 毒性T细胞的凋亡减少, 而毒性T细胞的活力提高, 认为: T细胞的删除, 导致肝的自然不被排斥; 作用于移植物的毒性T细胞的凋亡是引起小鼠肝移植分离免疫耐受的基本机制. Meyer et al[28]用大鼠和小鼠进行肝和小肠移植后认为: 细胞凋亡既在排斥反应中出现, 又在免疫耐受中出现; T淋巴细胞的激活与移植物实质细胞的凋亡有关, T细胞的失活似乎导致毒性T细胞的凋亡和特殊的免疫耐受; 细胞凋亡可以作为调节免疫的一个仪器.

Fas/FasL途径是细胞凋亡的重要调节方式. Famularo et al[29]报道Fas/FasL的相互作用是克隆删除、免疫耐受、赦免、在免疫反应中T细胞增生的控制、毒性T细胞的杀伤一个主要机制, 确信通过调节Fas/FasL的表达必将适合治疗与此有关的免疫参与的疾病. Tagawa et al[30]报道由Fas导致肝细胞凋亡引起ConA-肝炎模型中, IFN-r起中心作用. Solary et al[31] 研究肿瘤与细胞凋亡的关系时, 报道FasL的表达数量决定肿瘤细胞是被机体清除还是耐受. 低剂量的细胞毒性药物增加肿瘤细胞Fas表达, 从而改善了通过免疫细胞清除肿瘤细胞. Li et al[32]报道Fas对同种移植物的耐受不是必不可少的. 在IL-2依赖性的同种移植物免疫耐受机制中,Fas介导的细胞凋亡不是中心环节. Li et al[33]报道通过FasL基因转染的肝移植,观察大鼠的生存期明显延长,认为FasL表达的移植肝被受者免疫系统所保护. 国内外学者普遍认为角膜FasL的表达对于维持角膜免疫赦免具有非常重要的作用[34],其可能机制为: Fas与FasL相互作用引起T淋巴细胞凋亡, 降低免疫排斥反应发生率.

3.2 T细胞的凋亡导致免疫耐受

Bohmig et al[35]用一种短链脂肪酸下调了抗原提呈细胞(APC)的共刺激分子的表达, 使T细胞不能活化, 导致T细胞凋亡. Pender et al[36]提出: 在靶器官中对自身的和对同种反应的T细胞活化诱导细胞凋亡是免疫耐受的主要发生机制, 进一步分析指出: 已活化的自身反应T细胞当进入靶器官时, 主要由非专职APC使之再活化, 在产生明显的靶器官损害之前通过Fas (CD95)途径由活化诱导凋亡而被删除, 这是因为与Bcl-2相关的抗凋亡蛋白通过抑制CD95途径引起细胞凋亡, 再活化的T细胞不能从抗凋亡蛋白的表达已上调的非专职APC获得足够的共刺激分子. 而在周围淋巴器官, 情况相反, T细胞由专职APC再活化, 产生足够的共刺激分子, 导致抑制经CD95途径引起凋亡的与Bcl-2相关的蛋白表达上调, 从而允许T细胞以记忆T细胞存活、增值. 他用活化导致针对同种移植T细胞的凋亡来解释同种移植物的自然不被排斥, 也用以解释MHC不相符合的肝移植不发生排斥.

以上观点虽然能较好地解释器官移植的免疫耐受, 但不能解释皮肤、心脏、肾脏移植的耐受不如肝移植容易诱导和肝移植的特殊性. Crispe et al[37]认为肝脏是细胞死亡的"坟墓", 即肝脏是清除活化、凋亡T细胞的场所, 凋亡细胞在肝内的聚集是一主动过程, 肝脏的血窦内皮细胞(LSEC)、枯否氏细胞(KCs)和肝细胞均能够诱导细胞凋亡. Lu et al[38]发现移植肝内可迁徙的DC能表达FasL, 从而诱导同种异体反应的活化T细胞的凋亡. Weller et al[39]发现肝脏分泌的转化生长因子β(TGF-β)可以诱导T细胞凋亡, 可能也参与了调节T细胞的反应. 这些有助与解释肝移植免疫耐受的特殊性, 如"分离免疫耐受".

近年研究表明APC的抗原提呈能力不是一成不变的, 而是与APC的成熟程度和受刺激状况密切相关[40]. 在炎性因子作用下, APC的MHCⅡ合成增加、半衰期延长、结合抗原能力增强, 同时提供共刺激信号的能力大大加强[41-42]. 而在IL-10的作用下, APC共刺激信号弱表达并能诱导免疫耐受[43]. 这表明细胞因子在调节APC的抗原提呈能力及免疫反应方向等方面起着至关重要的作用.

在同种异体移植时给予供体凋亡细胞, 受体的APC在一些特异性的凋亡细胞的识别和结合受体如CD36、CD14、αvβ3 等的协助下迅速吞噬凋亡细胞, 以免细胞破裂损伤周围组织[44-46]. 我们推测凋亡细胞由于能快速被APC吞噬, 凋亡细胞上的抗原将被浓集在APC内, 因而能够短时提供大量供者抗原, 有效地被提呈; 而凋亡细胞通过其主动的调节免疫反应的作用, 使APC调节细胞因子由TH1型(如: IL-12、TNF-α)向TH2型(如: IL-10)转化, 并形成局部的高浓度. 凋亡细胞造成局部的免疫抑制环境, 使受体APC共刺激信号表达被抑制, 抑制了T细胞的激活, 使相应的T细胞克隆灭能, 从而诱发供体特异性的免疫耐受[47-48]. T细胞凋亡引起移植物免疫耐受的观点已得到多数学者的赞同.

最近Sun et al[48]提出了一个新观点: 肝脏是特异性地吞噬凋亡细胞的场所, 凋亡细胞有局部免疫抑制作用, 吞噬了凋亡细胞的肝脏抗原提呈细胞在局部免疫抑制的环境下提呈抗原给T细胞, 而诱导了对被吞噬抗原的免疫耐受. 以上观点能较好地解释肝移植免疫耐受的特殊性, 值得引起重视.

4 细胞凋亡与排斥反应

Krams et al[49]用大鼠肝移植模型, 检测肝酶、单核细胞肝脏浸润、肝脏病理、细胞凋亡变化后, 1995年首次提出在肝移植排斥反应中, 细胞凋亡可以是细胞坏死机制之一, 证实了以前描述的细胞因子在细胞凋亡中的作用在器官移植的排斥反应和免疫相关性疾病中, 细胞凋亡普遍存在[29,50-53], 排斥反应时, 器官组织的实质细胞发生凋亡.

5 细胞凋亡在肝移植免疫中的临床意义

在同种器官移植免疫中, 细胞凋亡发挥双刃剑作用. 当宿主T细胞凋亡, 可诱导免疫耐受; 另一方面, 免疫排斥反应发生时, 移植物细胞发生凋亡. 所以可从两方面着手为移植排斥反应防治提供新的策略: (1)预防移植物细胞凋亡, 减少移植物损伤; (2)促进受体反应性T淋巴细胞凋亡, 维持免疫赦免、诱导移植耐受.

在同种器官移植中某些高表达FasL的器官(如: 角膜、睾丸)易于诱导免疫耐受, 而其他一些器官则不易诱导耐受. 这一现象已引起各国学者注意, 并着手通过基因工程技术使某一器官特异表达FasL, 诱导免疫耐受[54]. 最近研究表明, 临床上部分新型药物可通过细胞凋亡途径发挥免疫抑制作用. Bcl-xl能阻止Fas介导的凋亡, 而FK506可通过下调Bcl-xl的表达明显增强T淋巴细胞的凋亡, 并可增强AICD的作用, 清除活化的T淋巴细胞, 发挥免疫抑制效能[55]. Migita et al[56]通过体外实验观察CsA和FK506对SEB诱导的活化T细胞的凋亡情况, 结果表明, FK506可下调Bcl-xl的表达, 促进T淋巴细胞的凋亡, 而CsA则不能. CsA可通过调节Bcl-2基因的表达, 使细胞毒T淋巴细胞发生凋亡, 从而防治移植抗原引发的排斥反应[57].

总之, 细胞凋亡在参与肝移植免疫耐受及移植排斥反应中具有重要意义. 对该领域的不断深入研究, 在器官移植, 尤其是肝移植排斥反应的防治、免疫耐受的诱导等方面前景广阔.
