The use of lasers to fuse different tissues has been studied for 50 years. As none of these experiments concerned the oral soft tissues, our objective was to assess the feasibility of laser gingiva welding.

Porcine full-thickness gingival flaps served to prepare calibrated samples in the middle of which a 2 cm long incision was closed, either by conventional suture or by laser tissue welding (LTW). To determine the irradiation conditions yielding the best tensile strength, 13 output power values, from 0.5 to 5 W, delivered either at 10 Hz or in continuous wave mode, were tested on six indocyanine green (ICG) concentrations, from 8% to 13% (588 samples). Then, some samples served to compare the tensile strength between the laser welded and the sutured gingiva; the other samples were histologically processed in order to evaluate the thermal damage extent. The temperature rise during the LTW was measured by thermocouples. Another group of 12 samples was used to measure the temperature elevation by thermal camera.

In the laser welding groups, the best tensile strength (p<0.05) was yielded by the 9% ICG saline solution (117 mM) at 4.5 W, 10 Hz, and a fluence of 31.3 kJ/cm(2). The apposition strength revealed no statistically significant difference (p<0.05) between the sutured and the laser welded gingiva at 4.5 W, 10 Hz, and 9% ICG solution. The mean temperature was 74±5.4°C at the upper surface and 42±8.9°C at the lower surface. The damaged zone averaged 333 μm at the upper surface.

The 808 nm diode laser associated with ICG can achieve oral mucosa LTW, which is conceivable as a promising technique of gingival repair.

Since the introduction of sutureless clear corneal cataract incisions, the procedure has gained increasing popularity worldwide because it offers several advantages over the traditional sutured scleral tunnels and limbal incisions. Some of these benefits include lack of conjunctival trauma, less discomfort and bleeding, absence of suture-induced astigmatism, and faster visual rehabilitation. However, an increasing incidence of postoperative endophthalmitis after clear corneal cataract surgery has been reported. Different authors have shown a significant increase up to 15-fold in the incidence of endophthalmitis following clear corneal incision compared to scleral tunnels. The aim of this report is to review the advantages and disadvantages of clear corneal incisions in cataract surgery, emphasizing on wound construction recommendations based on published literature.

Our aim is to explore the welding capabilities of a thulium (Tm:YAP) laser in modulated and continuous-wave (CW) modes of operation. The Tm:YAP laser system developed for this study includes a Tm:YAP laser resonator, diode laser driver, water chiller, modulation controller unit, and acquisition/control software. Full-thickness incisions on Wistar rat skin were welded by the Tm:YAP laser system at 100 mW and 5 s in both modulated and CW modes of operation (34.66 Wcm(2)). The skin samples were examined during a 21-day healing period by histology and tensile tests. The results were compared with the samples closed by conventional suture technique. For the laser groups, immediate closure at the surface layers of the incisions was observed. Full closures were observed for both modulated and CW modes of operation at day 4. The tensile forces for both modulated and CW modes of operation were found to be significantly higher than the values found by conventional suture technique. The 1980-nm Tm:YAP laser system operating in both modulated and CW modes maximizes the therapeutic effect while minimizing undesired side effects of laser tissue welding. Hence, it is a potentially important alternative tool to the conventional suturing technique.

Laser tissue welding was compared with a crude method of bipolar coagulator-generated heat application for achieving the same heat-induced welding effect in rat microarterial anastomoses. Rat femoral arteries were anastomosed with three triangulated stay sutures and subsequent laser welding or bipolar coagulator application between each pair of stitches. Control (non-welded) vessels received nine stitches placed circumferentially. Laser-welded vessel patency at 1 or more days postoperatively was 90% (65/72) for vessels treated with 0.1-second laser pulses, not significantly different from controls (100%; 16/16) or coagulator-welded anastomoses (88%; 14/16). Pseudoaneurysm rates were higher in the welded vessels (9% and 14% for laser- and coagulator-treated vessels, respectively) than in controls (0%). Histologic and electron microscopic evaluation revealed good healing with no apparent differences between laser- and coagulator-welded repairs. These findings suggest that laser application for microvascular tissue welding is similar to poorly controlled welding with a bipolar coagulator.

Multiple biliary papillomatosis is a rare tumour of the biliary tract but is capable of multicentric malignant transformation. The complete removal of these tumours is difficult because it involves a field change of the biliary tract.

Exploration with choledochoscopy is a prerequisite for thorough assessment of the extent of involvement. Incorporation of laser ablation via a laser-delivering fibre with choledochoscopy permits complete removal of the tumour.

Repeated laser therapy via choledochoscopy through the T-tube tract enables complete ablation of the tumour which changes the outlook of the prognosis. This is confirmed on subsequent cholangiography.

Laser therapy via choledochoscopy is a satisfactory mode of treatment for multiple biliary papillomatosis. It enables complete and precise ablation of the tumour which certainly changes the prognosis of this condition.

Postoperative bronchopleural fistula is a serious complication following pulmonary resection. To close the bronchopleural fistula, we developed a new method of endoscopic patch welding using laser tissue welding between bronchial tissue and a patch.

The feasibility of the laser patch welding was examined by in vitro and in vivo animal experiments. A newly developed carbon monoxide (CO) laser and an Argon ion laser were evaluated. Various tissue membranes and artificial membranes were evaluated as patch materials.

We found that the combination of expanded polytetrafluoroethylene (e-PTFE; 200 microns in thickness) and the CO laser with contact irradiation method offered the strongest laser patch welding. Using this combination, the irradiation at 400 W/cm2 for 10 seconds resulted in 16-18 g of measured traction strength at the welding spot (2 mm in diameter). The welded e-PTFE patch at bronchial stump remained for 5 weeks.

Our results encourage use of this novel laser patch technique for clinical applications.

The authors report six patients who had injuries to their common hepatic bile duct at laparoscopic cholecystectomy over a 16-month period. Five of the six complications could be attributed to laser injuries during dissection in the region of Calot's triangle. The authors discuss the possible mechanism of these injuries, their perioperative management, and the methods of surgical reconstruction. The follow-up period ranges from 3 months to 21 months. Liver function parameters and isotope biliary excretion scans are back to normal in all six patients. The potential hazards of laparoscopic surgery demand that extraordinary care be used not only during the actual surgical procedure, but also in the preoperative decision concerning the dissection method to be employed.

Laser welding offers several potential advantages over suture closure, including improved healing, lack of a nidus for stone formation, and greater speed and ease. We examined in vitro gallbladder cystic duct welds created by two different systems, the thulmium-holmium-chromium (THC):YAG (2,150 nm) and argon ion (488-514 nm) lasers, in an effort to define suitable parameters for tissue fusion. Mean bursting pressures for argon welds were 95 mm Hg at 1.5 W CW and 26 mm Hg at 1.5 W, 50 msec chopped delivery. For the THC:YAG laser, the mean bursting pressure for welds created with 300 mJ pulses was 45 mm Hg. Full-thickness tissue fusion and limited collateral thermal damage were observed histologically for both the CW argon and pulsed THC:YAG welds. Examination of the suggested mechanisms of tissue fusion for these photothermal lasers suggests that increased duration of tissue heating at the appropriate temperature results in more extensive collagen crosslinking and a stronger weld.
