PTH 300-399 pg/mL	0.50 mcg qd or 0.75 mcg qd once a day, 3 times a week
PTH 400-999 pg/mL	1.0 mcg qd once a day 3 times a week
PTH > 1000 pg/mL	2.0 mcg oral or IV 3 times a week
