炎症性肠病(inflammatory bowel disease, IBD)是一类肠道免疫系统过度激活引起的慢性非特异性炎症性疾病, 包括溃疡性结肠炎(ulcerative colitis, UC)与克罗恩病(Crohn's disease, CD). 免疫抑制剂在IBD患者中广泛应用, 且对于IBD治疗有效, 但是这些药物诱发淋巴瘤的发生报道越来越多, 尤其以硫唑嘌呤(azathioprine, AZA)为甚. AZA治疗IBD是否使淋巴瘤风险增加引起越来越多的关注. 本文主要对于AZA在IBD临床应用、诱发淋巴瘤发生的可能机制、风险评估等进行讲述.

Inflammatory bowel disease (IBD) is a chronic, non-specific inflammatory disease of the intestine, characterized by excessive activation of the immune system. Immunosuppressive therapy has been widely and effectively used in IBD patients. However, the occurrence of lymphoma after immunosuppressive therapy for IBD, especially azathioprine, has been recently reported. This article reviews the clinical application of azathioprine in IBD, the possible mechanisms responsible for lymphoma induction by azathioprine, and the assessment of benefit and risk of immunosuppressive therapy for IBD.

0 引言

淋巴瘤是最早发现的血液系统恶性肿瘤之一, 其起源于淋巴结及淋巴组织, 可分为霍奇金淋巴瘤和非霍奇金淋巴瘤两大类. 硫唑嘌呤(azathioprine, AZA), 作为免疫抑制剂, 主要通过抑制T淋巴细胞来抑制免疫反应[1,2], 因此可用于预防器官移植排斥反应[3], 也可用于类风湿性关节炎[4]、自身免疫性肝炎[5]、炎症性肠病(inflammatory bowel disease, IBD)[6]等自身免疫性疾病的治疗. 2000年国际IBD研究组织(International Organization for the Study of Inflammatory Bowel Disease, IOIBD)发起一项大规模对于免疫抑制剂在IBD的临床应用的研究[7], 报道了AZA治疗IBD患者所产生的不良反应, 如诱发胰腺炎、淋巴瘤、白血病、结肠癌、肾癌等. 近年来AZA治疗IBD诱导淋巴瘤发生的临床报道越来越多, IBD患者服用AZA是否可使淋巴瘤发生风险增加受到广泛关注. 本文主要讨论AZA在IBD临床应用、代谢过程、药理学作用、是否增加淋巴瘤发生风险以及评估其在IBD应用的利与弊.

1 硫唑嘌呤的IBD临床应用及指征

AZA是否可诱导缓解溃疡性结肠炎(ulcerative colitis, UC)与克罗恩病(Crohn's disease, CD), 不同的研究组有不一样的意见. 一项基于AZA诱导缓解急性CD的5个随机对照试验[8-12]和急性UC的2个随机对照试验[13,14]的荟萃分析指出单独应用AZA能够成功诱导急性CD和UC的缓解仍缺乏足够的证据. 而一项荟萃分析[15]认为AZA可诱导缓解急性CD, 其中指出AZA或6-巯基嘌呤(6-mercaptopurine, 6-MP)诱导缓解急性CD与安慰剂治疗比较, 其OR值为2.43(95%CI: 1.62-3.64), 需要治疗的人数(number needed to treat, NNT)大约为5, 如果剔除6-MP治疗CD数, 其OR值为2.06(95%CI: 1.25-3.39), 但是过敏、白细胞减少症、胰腺炎和恶心等不良反应发生增加, OR值为3.44(95%CI: 1.52-7.77), NTT为14. 然而其最近更新荟萃分析认为与安慰剂相比, AZA或6-MP对于活动性CD的诱导缓解或临床症状改善没有任何优势[16]. Gisbert等[17]进行一项荟萃分析, 其中纳入30项非对照试验, AZA/MP诱导缓解UC平均疗效率为65%; 而4项AZA/MP诱导缓解UC对照试验研究表明, 平均疗效达73%, 对照组为64%, 其OR值为1.59(95%CI: 0.59-4.29), 故与对照组相比, 仍不能认为AZA/MP可以有效地诱导缓解UC. 因此大多数学者认为单独应用AZA不能有效地诱导缓解急性CD和UC患者.

一般认为AZA可有效地维持缓解CD和UC患者. 最近一项更新荟萃分析[18]指出AZA有效地维持缓解CD治疗, 其Peto OR值为2.32(95%CI: 1.55-3.49), NNT为6, 且高剂量AZA能够更好地维持缓解; 同时报告对于类固醇依赖的CD患者, 应用AZA并撤退类固醇激素可有效地维持缓解CD患者, Peto OR值为5.22(95%CI: 1.06-25.68), NNT为3. 根据一项随机双盲对照试验[19], 建议AZA维持治疗CD持续时间应>3.5年. 一项关于AZA治疗UC疗效的荟萃分析[17]指出与安慰剂或5-ASA组相比, AZA可以有效地维持缓解UC, OR值为2.56(95%CI: 1.51-4.34). Peyrin-Biroulet等[20]意大利学者发起一项多中心观察性回顾研究, 其中建议AZA维持缓解UC持续时间应>6 mo, 而也有研究指出其持续时间应>18 mo[21], 故AZA治疗持续多久才能有效地维持缓解UC仍需进一步研究. AZA在IBD临床应用如表1所示[22].

2 硫唑嘌呤的代谢途径及药理作用

AZA发挥各种生物学效应, 主要通过转变为有活性的6-硫鸟嘌呤核苷酸(6-Thioguanine nucleotide, 6-TGN)实现的. AZA代谢途径[23]如图1所示, 大约90%AZA在肝脏非酶作用下快速裂解转变为6-MP, 6-MP经过以下复杂的生物转化转变为有活性或无活性的代谢产物: (1)6-MP经黄嘌呤氧化酶(xanthine oxidase, XO)催化形成6-硫脲酸(6-thiouric acid, 6-TU), 最后由肾脏排出体外; (2)6-MP在巯基嘌呤甲基转移酶(thiopurine methyltransferase, TPMT)甲基化作用变为非活性代谢物6-MMP; (3)此外, 6-MP可经次黄嘌呤磷酸酶(hypoxanthine phosphoribosyl transferase, HPRT)催化, 形成的第一个有活性的中间代谢产物单磷酸硫肌苷, 进一步催化形6-TGN, 还可转变为6-硫代鸟嘌呤磷酸(6-thioguanine diphosphate, 6-TGDP)和6-硫代鸟嘌呤三磷酸(6-thioguanine triphosphate, 6-TGTP)等.

一项前瞻性横断面研究[24]指出6-TGN推荐治疗阈值≥235 pmol/8×108红细胞, 且6-TGN水平低于或高于治疗阈值更可能分别与IBD活动期和缓解期有关. 然而6-TGN溶度过高可使骨髓抑制风险增高[25]. 活性代谢6-TGN在细胞中缓慢累积, 发挥抗炎作用至少可能需10-12 wk, 故AZA一般不单独应用于急性UC和CD患者诱导缓解治疗[26]. 然而, 其具体药理作用机制仍未完全阐明. 细胞内6-TGN可通过抑制DNA的合成和下游的T细胞增殖来抑制免疫反应[27-29]. 运用全基因组表达谱的方法[30], 6-TGN被发现能抑制多种免疫和炎症相关的基因, 包括抑制活化T淋巴细胞肿瘤坏死因子相关凋亡诱导配体、肿瘤坏死因子受体超家族成员7、α4-整合素. 另一份报告发现, AZA发挥免疫抑制作用部分是由于其代谢物6-TGTP抑制Rac1的蛋白质, Rac1参与T细胞的成熟和增殖, 从而诱导T淋巴细胞凋亡[2].

AZA诱导淋巴瘤、白血病、皮肤癌等恶性肿瘤发生, 其机制可能是: 6-TGN结构相似鸟嘌呤, 可掺入核糖核苷酸, 可能造成碱基的不稳定、错配, 暴露于致突变因素如紫外线、氧自由基等后可能诱发基因突变, 他们也可能直接抑制细胞毒性T细胞和天然杀伤细胞的功能, 抑制细胞介导的免疫监视, 使得EB病毒(Epstein-Barr virus, EBV)、人乳头瘤病毒等潜在感染的淋巴细胞大量增殖, 这将有助于发展淋巴组织增生性疾病及淋巴瘤[31-33].

3 硫唑嘌呤诱发IBD患者淋巴瘤的证据
3.1 IBD本身与淋巴瘤风险关系

一般认为IBD患者淋巴瘤的发病率与其他对照人群的发病率大致相同或略为增高[34,35]. Lewis等[34]进行回顾性队列研究, 其成功纳入了6605例CD患者和10391例UC患者, 对照组60506例, 与普通人群相比, 其相对危险度(relative risk, RR)为1.2, 95%CI: 0.67-2.06. Askling等[35]开展大型群体前瞻性研究研究发现UC中淋巴瘤的标准化发病率为1.0(95%CI: 0.8-1.3), 而CD为1.3(95%Cl: 1.0-1.6), 只在随访第一年呈边缘升高. 最近一个荷兰研究小组报告进一步证实IBD患者淋巴瘤发病率与同地域的其他人群没有差别[36]. 目前只有一个研究指出IBD与淋巴瘤发生有关, 且限于男性CD患者, 其可能与参照偏移、非霍奇金淋巴瘤及霍奇金淋巴瘤在人群中发病率不同有关[37].

3.2 硫唑嘌呤诱导IBD病情缓解治疗

最近一项对于AZA诱导缓解急性CD患者疗效的荟萃分析[16], 其中指出其主要不良反应为过敏反应, 白细胞减少症、胰腺炎和恶心, 但没有淋巴瘤报道, 这可能与AZA服用时间较短有关. 由于AZA治疗4-6 mo可能也不能获得最大抗炎作用[38], AZA一般较少单独用于急性UC和CD诱导缓解治疗, 因此对于AZA诱导缓解IBD患者是否可使淋巴瘤风险增加研究较少.

3.3 硫唑嘌呤维持IBD病情缓解治疗

一般认为AZA长时间维持缓解治疗IBD患者可使淋巴瘤发生风险增加. 一项大型前瞻性研究[39], 成功纳入IBD患者19486例, 其中CD 11759例、UC或未分型的IBD 7727例, 随访中位期为35 mo(29-40 mo), 其中连续接受AZA治疗的IBD患者5867例(30.1%), 间断接受AZA治疗2809例(14.4%), 未接受AZA治疗18810例(55.5%), 共确诊淋巴瘤23例, 其中22例为非霍奇金淋巴瘤, 对于连续接受AZA治疗的IBD患者, 每1000人年的淋巴瘤发生率为0.9(95%CI: 0.50-1.49), 间断接受AZA治疗每1000人年的淋巴瘤发生率为0.2(0.02-0.72), 而未接受AZA治疗每1000人年的淋巴瘤发生率为0.26(0.10-0.57), 上述3组间存在显著差异(P<0.05), 且AZA治疗诱发淋巴瘤发生整体多元变量危险比为5.28(95%CI: 2.01-13.9); 对于服用AZA, <50岁年龄组每1000人年淋巴瘤发生率为0.37%, 而>65岁年龄组为5.41. 前期研究结果表明AZA治疗IBD患者可使淋巴瘤发生风险增加, 且如果患者不再服用AZA, 淋巴瘤的发病率与从来没有AZA治疗的患者相比没有明显增高, 淋巴瘤发生风险增加可能还与年龄有关.

包括6个队列研究的荟萃分析[40]认为长期服用AZA治疗使得淋巴瘤风险增加, 且年龄对于淋巴瘤发生率有影响, 研究中发现淋巴瘤11例, 而预期数为2.63例, 因此集中标准化率(standardized incidence ratios, SIR)为4.18(95%CI: 2.07-7.51); 其中敏感性分析指出排除对于集中相对危险度影响较小的研究, 重新计算得出SIR(3.49-5.21); 且发现年龄对于淋巴瘤发生率有影响, 年龄越大, 淋巴瘤发生率越高, 从20-29岁大约为1/4357至70-79岁为1/355.

研究指出长期服用AZA维持缓解治疗IBD诱导淋巴瘤发生, 大部分病例病理组织学上与移植术后淋巴瘤表现一致, 主要来源于B细胞, 且60%-70%的病例是EB病毒阳性[39], 约5%移植术后淋巴瘤来源于T细胞和T/NK细胞[41], 这可能与AZA抑制免疫监视功能, 使得潜在EBV感染细胞克隆性增殖有关. EBV阳性淋巴瘤和AZA明显相关性在荷兰最近的一项全国性研究中也观察到[36]. 这都为AZA使淋巴瘤风险增加提供了证据.

3.4 硫唑嘌呤治疗后停药

有报道指出长期服用AZA治疗IBD并发淋巴瘤后停药4 wk后淋巴瘤明显好转, 1年后淋巴瘤完全消退[42]. 在系统性红斑狼疮合并淋巴瘤患者也发现停用AZA后淋巴瘤消退[43]. 一项法国多中心的荟萃分析指出患者不再服用AZA, 淋巴瘤的发病率与从来没有AZA治疗的患者相比没有明显增高[39]. 停止AZA后淋巴瘤消退这一现象可能与停药后免疫功能得以恢复有关. 然而, IBD合并淋巴瘤患者停用AZA后淋巴瘤消退临床报道较少, 故仍需进一步研究.

4 硫唑嘌呤临床应用利与弊

AZA虽有众多不良反应, 如急性胰腺炎、白细胞减少、白血病、皮肤癌, 然而, 与未服用者相比, 整体恶性肿瘤发生率并没有增高[44]. AZA可能使淋巴瘤发生风险增加约4倍[40], 但早期长期AZA治疗IBD有着较大有益作用, 能够有效地维持缓解及预防IBD复发, 并可降低外科手术率.

有报告指出对于儿童IBD患者, 早期更系统的AZA治疗可以使得缓解时间延长2倍[45]. 来自匈牙利的成人IBD患者队列研究报告AZA治疗长期疗效与疾病行为的变化和切除手术风险降低是独立相关的. 他还防止吸烟的有害影响[46,47]. 这也在美国一项回顾性队列研究中得到证实[48], 非穿通性非炎症性CD患者使用AZA时间超过6 mo, 其外科手术风险危险比(hazard ratio, HR)为0.41(95%CI: 0.21-0.81). 为了研究停用AZA后IBD复发, 有一项研究纳入缓解期CD患者66例, 发现停用AZA后在36 mo内32例患者疾病复发, 且复发患者中23例重新给予AZA治疗仅1例获得临床缓解[49]. 一项多中心回顾研究[50]进一步证实维持缓解及预防UC复发, 并可降低外科手术率, 127例缓解期UC患者, 随访中位数为55 mo, 发现停用AZA后1年复发率为1/3, 2年为1/2, 而5年疾病均复发, 疾病复发预测因素与缺乏AZA维持缓解治疗(HR: 2.350, 95%CI: 1.434-3.852)及治疗持续时间(3-6 mo短期治疗比>48 mo治疗更不利, HR: 2.783, 95%CI: 1.267-6.114)有关; 停用AZA后结肠切除率为10%, 且其预测因素与停用AZA引起的药物相关不良反应有关, 而与服用AZA后不良反应和持续时间无关. 使用马尔可夫模型的一项决策分析表明AZA用于维持缓解CD患者治疗利大于弊, 整体调整质量寿命期望值增加, 这种增长在年轻患者中尤为明显[51].

5 结论

AZA一般需10-12 wk才能发挥其治疗效应, 故一般不用于急性UC和CD治疗, 但能够有效地维持缓解、预防IBD复发及降低外科手术率. AZA可诱发皮肤癌、直结肠癌、白血病等致死性恶性肿瘤发生, 但与未服用者相比, 其整体风险并未增加. AZA虽可使淋巴瘤发生率增加4倍, 但停药后淋巴瘤可能消退, 且AZA诱导淋巴瘤增加的绝对风险较低, 使用马尔可夫模型的一项决策分析表明AZA治疗利大于弊的, 因此AZA使淋巴瘤风险增加4倍不能阻止AZA用于IBD患者治疗. 然而, AZA诱导淋巴瘤发生具体机制, 如何预防及监测淋巴瘤发生, AZA停药后淋巴瘤的消退及转归, 合并淋巴瘤发生后治疗等问题仍需进一步研究.
